Position Purpose

The Foundation for Ageing and Inclusive Development (FAID) is a national organization dedicated to promoting inclusive development, enabling people of all ages—especially older people—to lead dignified, healthy, and secure lives. FAID will implement the project titled “AGE-CARE: Advancing Geriatric Excellence through Community-based ICOPE and NCD Integration in Khyber Pakhtunkhwa,” with technical support from HelpAge International and funding from the SIDA.

 

This 12-month initiative will be carried out in close collaboration with the Ministry of National Health Services, the Health Department of Khyber Pakhtunkhwa, and the World Health Organization (WHO). The project aims to improve both policy and service delivery for older people by combining national-level advocacy with district-level implementation of WHO’s Integrated Care for Older People (ICOPE) model, integrated with the prevention, management, and treatment of Non-Communicable Diseases (NCDs).

 

The project includes three key outcomes:

  1. Finalizing and validating the national Healthy Ageing Framework through inclusive consultation processes.
  2. Piloting the ICOPE approach in Nowshera district of KP through health facility strengthening, community screening, and training of health professionals and facilitators.
  3. Advocating for the integration of ICOPE and NCD care into KP’s provincial health systems and Universal Health Coverage framework.

 

The Social Mobilizer will play a vital role in the implementation of the AGE-CARE project, working at the grassroots level to engage and empower communities, especially older people and their representative bodies such as Older People’s Associations (OPAs). This position is central to ensuring the effective rollout of WHO’s ICOPE (Integrated Care for Older People) model and the integration of Non-Communicable Disease (NCD) prevention and management services in the target district of Khyber Pakhtunkhwa. The Social Mobilizer will be responsible for the mobilization, coordination, and facilitation of community-based activities, including raising awareness about healthy ageing, supporting the identification and screening of older people, and ensuring strong linkages between OPAs, trained community facilitators, and designated health facilities piloting the ICOPE approach. This role also involves building relationships with health facility staff to facilitate seamless referrals, community-health system coordination, and follow-up support for older individuals identified during community screenings. The Social Mobilizer will serve as a critical link between the community and health service providers to ensure that interventions are inclusive, accessible, and responsive to the specific needs of older people, including those with disabilities. By supporting both community engagement and coordination with service providers, the Social Mobilizer will help ensure that the pilot implementation of ICOPE is community-driven, well-integrated with the local health system, and aligned with the overall goals of Universal Health Coverage (UHC) for older people.

 

Key Responsibilities:

The key responsibilities of Social Mobilizers are included:

1-     Community Mobilization and Engagement

-         Mobilize and engage communities, particularly older people and their families, to raise awareness about healthy ageing, NCD prevention, and the benefits of integrated care.

-         Support the identification, activation, and capacity strengthening of Older People’s Associations (OPAs) as key community structures for the project.

-         Ensure representation and meaningful participation of older people, including women and persons with disabilities, in all community-level activities.

2-     Support ICOPE Screening and Follow-up

-         Coordinate and support the work of trained community facilitators responsible for conducting ICOPE-based screenings of older individuals.

-         Facilitate the development and tracking of individual care plans for older people screened at the community level.

-         Ensure accurate documentation and reporting of screening data, referrals, and follow-up outcomes.

3-     Coordination with targeted Health Facilities

-         Build and maintain effective working relationships with the staff of designated health facilities involved in the ICOPE pilot.

-         Facilitate community-health facility coordination, including referrals and feedback mechanisms to ensure continuity of care.

-         Support the integration of community-level inputs into health facility services, particularly age-friendly care.

4-     Assistive Technology and Referral Support

-         Identify older people in need of assistive devices (e.g., walking aids, hearing aids) based on screening results.

-         Facilitate the timely and transparent distribution of assistive technologies and provide information about usage and maintenance.

-         Support follow-up referrals for specialized care, diagnostics, or rehabilitation as needed.

5-     Awareness and Education Activities

-         Organize community education sessions, health awareness events, and local campaigns aligned with national and international observance days (e.g., International Day of Older Persons).

-         Disseminate informational materials, videos, and tools on ICOPE, NCDs, and healthy ageing practices.

6-     Inclusion, Safeguarding, and Do No Harm

-         Promote inclusive practices in all interactions and ensure the active participation of marginalized groups.

-         Adhere to FAID’s safeguarding and protection policies, reporting any incidents or concerns promptly.

-         Promote a respectful, safe, and inclusive environment for all beneficiaries and team members.

 

Specific duties and responsibilities

The specific duties and responsibilities for the position holder are including:

  1. Identify, engage, and maintain regular communication with 25 OPAs in the target district to support their participation in project activities.
  2. Assist in the selection, orientation, and coordination of 100 community facilitators (male and female) who will conduct ICOPE and NCD screenings.
  3. Facilitate and monitor community-based screenings of 1,500 older people using the ICOPE tool, ensuring gender and disability-sensitive approaches.
  4. Provide field-level support during the distribution of 500 assistive devices, ensuring that items are need-based, appropriately documented, and accompanied by usage guidance.
  5. Serve as a focal point between community structures and the 8–10 designated health facilities, ensuring smooth coordination of referrals, data sharing, and care follow-up.
  6. Support the establishment and functioning of Age-Friendly Care Corners by identifying community needs and ensuring beneficiary awareness of available services.
  7. Coordinate logistics and outreach for community-level training sessions, awareness campaigns, and district events related to healthy ageing and NCDs.
  8. Conduct regular home visits and follow-ups for older people who have been screened and referred, to monitor progress on care plans and identify unmet needs.
  9. Gather and document case studies, success stories, and beneficiary feedback, and submit them as part of learning and monitoring efforts.
  10. Identify and report any protection or safeguarding concerns, especially those affecting older people, women, or persons with disabilities, using established FAID protocols.
  11. Support the planning and facilitation of community-based review and reflection meetings to gather community feedback and improve activity implementation.
  12. Liaise with local leaders, religious figures, and service providers to build community ownership and accept the ICOPE model and NCD care.
  13. Assist with logistical arrangements for community-based events, including local awareness drives, International Day of Older Persons activities, and video dissemination sessions.
  14. Ensure that all activities are well-documented, with timely submission of attendance sheets, field reports, and referral logs.
  15. Maintain confidentiality and handle all personal data of beneficiaries responsibly and in line with FAID’s data protection policies.

Required qualifications and experience

ü  Bachelor’s degree in social work, Sociology, or equivalent.

ü  2–3 years’ experience in community mobilization or health projects.

ü  Strong interpersonal and communication skills.

ü  Fluency in local languages is preferred.

ü  Ability to work in team and network with partners

ü  Good skills of computer
